The Quality and Technical Assistance Center

Through its Quality and Technical Assistance Center (QTAC), the Center for Excellence in Aging & Community Wellness supports the building and sustaining of local capacity throughout the state to deliver evidence based health promotion and disease prevention programs. Although it provides technical assistance across a wide range of evidence-based programs, the QTAC is most actively involved in the statewide implementation of the Chronic Disease Self-Management Program in New York State.

Statewide Implementation of the Chronic Disease Self-Management Program (CDSMP)

Specifically, the QTAC provides technical support to New York localities through:

  • Training and certification of local “master trainers” and “peer leaders,” utilizing a “train the trainer” approach to evidence-based program dissemination
  • Training and support for regional/local program staff in quality assurance/quality improvement techniques, fidelity monitoring and management, and data collection protocols
  • Healthy Choices NY Learning CommunityUsing its web-based community learning sites, resource toolboxes, support forums, webinars and other technical assistance activities to connect communities to the resources and best practice strategies needed for better participant and peer leader recruitment, program implementation and evaluation, quality assurance/quality improvement and sustainability
  • Managing quality related data collection and reporting to funders regarding participant outcomes, systems change, and stakeholder satisfaction.
