Living Healthy New York
For the Recovery Act Chronic Disease Self-Management Program, please see the Healthy Choices NY page.
Living Healthy* is a six week workshop that teaches practical skills while dealing with chronic conditions such as diabetes, heart disease, obesity, and arthritis. If you or someone you care for has one of these conditions, the Living Healthy Workshop can help you learn the tools for better self-management.

“After taking the class I’m now a problem-solver instead of a victim, and I can communicate easier with my doctor about my needs.”
These self-management tools will help you:
- Find practical ways to deal with fatigue and pain.
- Discover better nutrition and exercise choices.
- Understand treatment choices.
- Learn better ways to talk with your doctor and family about your health.
- Learn real-life skills for living a full, healthy life with a chronic condition.

“These weekly goals motivated me…to do things that I didn’t think I could do. I would recommend this class to anyone with a chronic condition.”
Workshops
The way the program is taught is what makes it effective. The sessions are lively and participative: Workshop participants talk, share practice skills and learn together over the six weeks. Mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives. “I would like you to know how much I have gotten out of this six-week course. Being goal-oriented has helped me in planning my week. I have been more alert after exercising at least three times a week—swimming and walking. Thank you!”
