<p>The Living Healthy Workshops are a FREE workshop series, offering six weekly 2.5 hour sessions designed to help older adults with a Chronic Condition such as Heart Disease, Arthritis, Diabetes, Asthma, and others and/or Caregivers. Each registered participant that attends the workshop receives a Free workbook and relaxation CD. A light snack is also provided.</p>

			<content:encoded><![CDATA[<p></p><h3>The Center for Excellence in Aging &amp; Community Wellness at the State University of New York at Albany, School of Social Welfare Will Receive Funding from New York State to Provide Evaluation and Technical Assistance for the Implementation of the Stanford University-based Chronic Disease Self-Management Programs</h3>
<h4>Funds Will Support Self-Care Community Workshops for 5,000 Older Adults Throughout New York State</h4>
<p>Albany, September 14, 2010 – The Center for Excellence in Aging &amp; Community Wellness at the State University of New York at Albany, School of Social Welfare has received funds to assist in building an infrastructure for the delivery of evidence-based health promotion programs in New York State communities helping those with chronic conditions, such as arthritis, cancer, or diabetes to improve their quality of life.</p>
<p>The Center for Excellence in Aging and Community Wellness is part of a grant through the American Recovery and Reinvestment Act (Recovery Act), issued by the U.S. Administration on Aging.</p>
<p>Living Healthy NY workshops bring people with chronic diseases, family members, and caregivers together in a community setting to learn healthier ways to live, gain confidence and motivation to manage their health, and feel more positive about life.</p>
<p>The workshops are highly interactive and provide the tools needed to manage symptoms, work with health care professionals, set weekly goals for behavior change, problem-solve, use relaxation techniques, handle difficult emotions, eat well, and exercise safely and easily.  The workshop is not disease-specific, but it is designed to support and enhance disease-specific education.</p>
<p>“We are excited to bring this successful program into our New York State community,” said Phillip McCallion, PhD, the Center’s Director. “The program has been extensively researched and shown to be effective in reducing the symptoms associated with many chronic conditions and in some cases in reducing health care costs.”</p>
<p>“Our goal in New York is to provide the Living Healthy workshop to 5,000 citizens in the next 18 months.  We are looking for citizens of New York who want to take charge of their health and become part of a State and national movement. The infrastructure we are building supports the quality delivery of this and other evidence-based programs that empower New York’s citizens to live healthier lives.” – Lisa A Ferretti, Director New York State Chronic Disease Self-Management Quality and Technical Assistance Center.</p>
